Job summary

EAST LONDON NHS FOUNDATION TRUST is seeking an experienced Community Nurse to join the Unplanned Care Night Team as a part-time Band 6 nurse. The role focuses on triage, assessment, and urgent care for housebound patients, with responsibility for cross-cover between North and South Bedfordshire.

You will conduct home visits, manage End of Life Care and catheter care where needed, and liaise with Out-of-Hours GP and palliative services while traveling across Bedfordshire with a driving license.

Qualifications

• Experienced community nurse with ability to work night shifts.
• Excellent communication and time management skills.
• Sound clinical knowledge with ability to triage and assess patients.
• Flexible approach to cross-cover in Bedfordshire.

Responsibilities

• Triage referrals and carry out initial history taking and clinical assessment.
• Provide nursing care during face-to-face home visits to housebound patients.
• Manage End of Life Care needs and catheter care where required.
• Liaise with Out of Hours GP and Bedfordshire Palliative Care Services.
• Cross-cover between North and South Bedfordshire as needed.
• Work 20:00–08:00, 7 days a week, with driving between areas.
