Job summary

University Hospitals of Morecambe Bay is recruiting a Band 5 Nurse to join the North Lancs Rapid Response team in the community to cover maternity leave on a fixed-term contract for up to 12 months. You will support the urgent community response standard and be part of a multidisciplinary team delivering a 2-hour crisis response to acutely unwell patients to prevent hospital admissions.

You will have excellent communication skills and be able to work autonomously as well as in a team.

Qualifications

• Registered nurse with Band 5 level in adult care.
• Experience in high-pressure community/urgent care settings.
• Proven ability to assess patients holistically and develop care plans.

Responsibilities

• Provide holistic assessments in challenging situations.
• Deliver robust care programs for patients and carers.
• Collaborate with multidisciplinary teams to prevent unnecessary hospital admissions.
