Job summary

University Hospitals of Morecambe Bay NHS Foundation Trust is seeking a Band 5 Rapid Response Nurse to join the community team in Heysham on a fixed-term contract to cover maternity leave. This part-time role (30 hours per week) focuses on urgent community care and a 2-hour crisis response to prevent hospital admissions.

You will work within a multidisciplinary team, undertake holistic assessments, and contribute to a patient-centered service with structured induction and flexible working

Qualifications

• Band 5 Registered Nurse (adult) with current UK registration.
• Ability to undertake holistic assessments in community settings.
• Excellent communication and interpersonal skills.

Responsibilities

• Support delivery of the urgent community response standard.
• Provide a 2-hour crisis response to acutely unwell patients.
• Collaborate with multidisciplinary team to prevent hospital admissions.
