Job summary

St. Luke's Hospice seeks a Rapid Access to discharge (RADS) Nurse Clinician to assess and support patients nearing end of life, coordinating with carers and external providers to ensure preferred place of care.

The role involves MDT collaboration, holistic assessment, information provision, and liaising with health and social care professionals to enable patient choices and timely care at home or in community settings.

Qualifications

• Registered Nurse with current registration and relevant clinical experience.
• Excellent IT, organisation and communication skills.
• Car driver with vehicle for work purposes.

Responsibilities

• Provide holistic assessments and coordinate palliative care in patient homes or with domiciliary providers.
• Work within a MDT to deliver holistic end-of-life care and support families/caregivers.
• Coordinate placements with care homes and domiciliary providers, monitoring and reviewing arrangements.
