Job summary

St Luke's Hospice in Basildon is seeking an experienced Palliative Care Clinical Nurse Specialist with a passion for working in a charity focused on compassionate care.

The role is 37.5 hours per week over 4 days, with 8am-6pm shifts, covering on-site Basildon and community work. You will lead specialist palliative care, manage complex caseloads, collaborate with primary and secondary care, and contribute to MDTs.

Qualifications

• Must be a first level registered nurse.
• Significant post-registration palliative care experience at Band 6/7 or above.
• Ability to work alone in the community and manage complex cases.
• Expert knowledge of pain and symptom management.
• Computer literacy to input patient records.
• Good liaison with professionals to uphold specialist palliative care.

Responsibilities

• Lead specialist and source of expertise in palliative care; provide strategic and clinical leadership.
• Manage patients on caseload with complex needs and support families.
• Promote integrated teamwork with partners and providers.
• Undertake holistic assessment and plan level of involvement.
• Prioritise workload and plan care in collaboration with other professionals.
• Coordinate care for admissions and discharges as appropriate.
• Provide high-quality information to patients and families.
• Contribute to MDTs and Gold Standard Framework meetings.
• Work within OneResponse team and other community services.
• Undertake post-bereavement visits when required.
• Develop and participate in services and meetings to meet goals.
• Advance care planning and ensure best interest decisions.
