Job summary

St Luke's Hospice is seeking a Rapid Access to discharge (RADS) Nurse Clinician to join our specialist Teams in Basildon. You will deliver holistic assessments and coordinate care for patients in their home or with 24-hour support, working with domiciliary providers and families.

The role requires nursing registration, significant end-of-life care experience, strong IT/communication skills, and driving ability to support rapid access services in the community.

Qualifications

• Registered Nurse with current license and registration.
• Experience in end-of-life or palliative care.
• Proficient IT skills and ability to use care coordination systems.
• Valid driving license with access to a vehicle.

Responsibilities

• Provide clinical care, navigation, coordination for hospice patients and families.
• Share caseload within the MDT across hospice community services.
• Use holistic assessments to advise on care and liaise with health and social care professionals.
• Help patients access health and social care information and support when needed.
• Coordinate placements with care homes or domiciliary providers and monitor.
• Build strong external partnerships to support coordinated care.
