Job summary

St Leonard’s Hospice in York invites applications for a Patient Flow Coordinator who will support the transfer of patients between hospice services. You will be a key point of contact for patients, families and professionals to guide these transfers to ensure the best care possible is received.

This full-time, 37.5-hour week role offers a salary of £28,555 to £31,337 per annum and a supportive team environment with opportunities for training, flexible days and NHS-style benefits.

Qualifications

• Experience in health or social care focusing on discharge and transfer of care.
• Excellent organisational and communication skills.
• Ability to stay calm under pressure and meet timescales.

Responsibilities

• Act as the first point of contact for Hospice services, handling complex information.
• Liaise with local commissioners for timely onward care.
• Work with Therapies team to review equipment needs.
