Job summary

The Royal Wolverhampton NHS Trust is seeking a Care Co-ordinator within the Rapid Access to Social Care service. You will assess, coordinate and facilitate health and social care support to ensure timely interventions and safe discharge planning.

Working in a multidisciplinary team, you will manage referrals, coordinate patient pathways, maintain accurate records and escalate risks, while delivering person-centred care and supporting efficient use of resources.

• Assess, coordinate and facilitate health and social care support for patients within the Rapid Access to Social Care service.
• Manage referrals and coordinate patient pathways with multidisciplinary teams.
• Maintain accurate records and identify escalations of risk.
• Support discharge planning and admission avoidance to improve patient outcomes.
