The Royal Wolverhampton NHS Trust is seeking a Care Co-ordinator for the Rapid Access to Social Care service. The post involves assessing referred patients and arranging care packages to enable home or post-discharge care, liaising with clinicians, carers and partner agencies.

Experience in health, social care or community care and NVQ Level 3 or equivalent is required. Excellent communication and IT skills, and the ability to supervise support staff, are essential for success in this role.

Aufgaben

• Undertake holistic assessments of referred patients, identifying health, social care and environmental needs.
• Coordinate, arrange and monitor care and support packages to keep patients safely at home or return them home.
• Manage and prioritise referrals, completing assessments and interventions within agreed timescales.
• Support discharge planning and admission avoidance pathways to improve patient flow.
• Act as key point of contact for patients, carers, families and partner agencies.
• Work with multidisciplinary teams, including nursing, therapy, social care, primary care and voluntary sector, and take part in MDT meetings and case discussions.
• Identify, assess and elevate risks, safeguarding concerns and changes in patient condition under Trust policy.
• Maintain accurate, timely and confidential patient records on electronic systems, in line with information governance requirements.
• Provide advice on services, support options and community resources to patients and carers.
• Monitor patient progress and review care arrangements against individual needs.
• Support day-to-day running of the service, including data collection, reporting and administration.
• Contribute to quality improvement, audit and performance monitoring.
