Job summary

The Royal Wolverhampton NHS Trust is seeking a Care Co-ordinator within the Rapid Access to Social Care service. You will assess, coordinate and facilitate health and social care support to ensure timely interventions and safe discharge planning.

Working in a multidisciplinary team, you will manage referrals, coordinate patient pathways, maintain accurate records and escalate risks, while delivering person-centred care and supporting efficient use of resources.

Job description

The Rapid Access to Social Care (RASC) Care Co-Ordinator plays a key role in supporting the delivery of timely, person-centered care by undertaking assessments, coordinating care and support packages, and facilitating safe discharge and admission avoidance pathways.

Working as part of a multidisciplinary team, the post holder will liaise with healthcare professionals, patients, carers and external agencies to ensure individuals receive the right support at the right time.

The post holder will be responsible for managing referrals, coordinating patient pathways, maintaining accurate records, identifying and escalating risks, and supporting the smooth day-to-day running of the service.

The role requires excellent communication, organisational and IT skills, with the ability to prioritise workload effectively in a busy and fast-paced environment. The Care Co-ordinator will contribute to improving patient outcomes, promoting independence and ensuring efficient use of health and social care resources., To assess, coordinate and facilitate health and social care support for patients within the Rapid Access to Social Care service, ensuring timely interventions, safe discharge planning, admission avoidance and effective communication with patients, carers and multidisciplinary teams.
