Our Team is Community Health and Social Care, Seven Kings Cluster (HASS Seven Kings) – an integrated health and social care service.

You will be working as part of an MDT with LD Health practitioners (including physiotherapists, occupational therapists, psychologists, psychiatrists, speech and language therapist and community nurses) and administrators.

The Team works with people with varied needs, from 18 years to the end of life. Practitioners' caseloads will reflect the varied needs accordingly.

You will be office-based at HASS Seven Kings - the working pattern is a hybrid system – 2/3 days in the office and the rest remotely. In addition, when we are on Duty (usually one week every 4/5 weeks), we are office-based for 5 days. This may sometimes change depending on service needs.

The work is intense and fast-paced. We seek social workers with robust experience working with people with varied needs. The suitable candidates must understand and effectively practice within statutory guidelines/framework – be independent, creative, work under pressure, be resilient, and deliver assessments and support plans within agreed timescales.

All assessments/reviews are completed face-to-face.

We provide support, training, and supervision. We provide a laptop and a smartphone. We are a friendly, warm, and welcoming team - continuity/stability is crucial.
