Sutton Primary Care Networks is recruiting a Patient Contact Coordinator to support the Proactive Anticipatory Care (PAC) Service. You will proactively contact identified patients, explain aims and benefits, address questions and encourage participation in proactive healthcare and wellbeing support.

In this role you will handle telephone enquiries, maintain accurate records, arrange appointments, and work with the multidisciplinary team to improve patient uptake and accessibility of the service.

Qualifications

• Experience in customer service, administration or patient-facing roles.
• Experience communicating with individuals in a sensitive and professional manner.
• Experience maintaining accurate records and databases.
• Experience managing telephone enquiries and communicating effectively over the telephone.

Responsabilités

• Proactively contact patients identified as benefiting from the PAC service.
• Explain the purpose and benefits of the PAC service in a patient-centred manner.
• Maintain accurate records and update patient tracking systems.
• Arrange appointments and assessments for PAC team members.
• Signpost callers to appropriate healthcare and support services.
• Escalate urgent clinical or safeguarding concerns per procedures.
