Job summary

Trinity Health Group, a Primary Care Network, is seeking a Care Co-ordinator to work with the PCN team to deliver personalised holistic healthcare and proactive support to patients in Wakefield.

You will identify patient cohorts using population health intelligence, develop care plans with families, signpost to services, and support MDT meetings, with training and supervision provided.

Qualifications

• Experience in care coordination or related healthcare role.
• Ability to interpret population health data to identify patient cohorts.
• Strong communication and signposting skills.

Responsibilities

• Identify cohorts of patients using population health intelligence and prioritise support.
• Develop and support personalised care plans with patients and families.
• Navigate and signpost to health, social care and community services; coordinate appointments.
• Support MDT meetings and vaccination programmes where required.
