Job summary

Taunton Deane West Primary Care Network is seeking an Administrative Care Coordinator to provide high quality administrative coordination and personalised care support within the GP practice.

You will work with GPs, Social Prescribing Link Workers, Health and Wellbeing Coaches and multidisciplinary teams to coordinate patient care, access services, and support personalised care planning across the PCN.

Qualifications

• 4 GCSEs level 4 and above including English and Maths.
• NVQ level 2 in health and social care desirable.
• Experience with data, databases or information systems.

Responsibilities

• Book appointments for patients requiring review through recall and screening systems.
• Process clinical documents, tasks, referrals and admin workflows.
• Manage incoming/outgoing correspondence; draft responses where appropriate.
• Maintain accurate patient records; ensure actions recorded and completed promptly.
• Summarise patient records and update clinical systems.
• Support processing of Subject Access Requests and information governance activities.
• Undertake general admin duties to support day-to-day operations of the practices and PCN.
• Coordinate patient care and support multidisciplinary team discussions and documentation.
