• Experience of working in a multidisciplinary team.
• Minimum of 5 years post-qualification experience.

Responsibilities

• Deliver Structured Medication Reviews (SMRs) and manage long-term conditions.
• Work closely with primary care colleagues and general practitioners.
• Contribute to quality improvement and clinical audit activities.

Skills

Excellent written and verbal communication skills

Experience of influencing others

Good clinical knowledge

Understanding of prescribing data

Education

Vocational master's degree in Pharmacy

Registered with the General Pharmaceutical Council
