The Princess Alexandra Hospital NHS Trust is seeking a proactive, patient‑facing postholder to support the Macmillan Recovery Package and risk-stratified follow‑up after cancer treatment. Qualifikationen

• Extensive patient-facing experience in healthcare settings.
• Knowledge of health and social care.
• Customer service skills training.
• Good standard of general education to GCSE level or equivalent.

Aufgaben

• Work with multidisciplinary teams to deliver Recovery Package and risk stratification processes, including holistic needs assessments, treatment summaries, wellbeing events and remote surveillance
• Act as the main point of contact across the patient journey and identify risk-stratified patients who should receive the Recovery Package
• Manage the cancer surveillance database with CNS teams and the cancer information manager, keeping records accurate and validated
• Support and co-ordinate patients on patient-triggered follow-up pathways, escalating concerns to the relevant practitioner
• Apply Macmillan level 1-2 interventions to support non-complex cancer patients
• Help patients complete electronic holistic needs assessments and develop care plans to address their concerns
• Co-ordinate post-treatment follow-up investigations and fast-track patients back into the specialist cancer team when needed
• Take and relay telephone calls, give basic telephone advice and signpost patients to other support
• Deliver health education on signs, symptoms and late treatment effects for non-complex patients
• Support the planning, delivery and evaluation of self-managed workshops and events
• Evaluate practice annually, collecting data for the Macmillan Annual Report and service user feedback
• Liaise with internal and external stakeholders and provide day-to-day administrative support and data entry
