Job summary

The Norfolk and Norwich University Hospitals NHS Foundation Trust is hiring a Macmillan Cancer Care Navigator to guide newly diagnosed patients through Holistic Needs Assessments, information and signposting to services. You will work with the Cancer Services team to support patients and their carers in navigating health and social care, with empathy and confidentiality.

Based at Rouen Road, you will engage with patients in person and by phone, help run information pods and outreach sessions,

Qualifications

• Direct newly diagnosed cancer patients through Holistic Needs Assessment (HNA).
• Provide information and signposting to relevant services.
• Support patients and carers in navigating the health and social care system.

Responsibilities

• Direct patients via telephone and face-to-face contact for HNA.
• Manning the NNUH Information Pod and Cromer Centre for drop-in information.
• Support outreach services in the community and health and wellbeing events.
• Visit patients within clinical areas and inpatients as needed.
• Communicate with patients and carers in clinical settings and on the telephone.
