Job summary

Coastal Fareham & Gosport Primary Care Network is seeking a Cancer Care Coordinator to act as a dedicated contact for patients after a cancer diagnosis, helping families access information and support. You will coordinate care across health and social care services for frail or elderly patients and support practices with screening uptake and referral quality.

The role involves assisting MDT meetings, monitoring pathways to meet Cancer Waiting Time Targets, and developing safety-netting

Qualifications

• Experience working with patients and carers through the early cancer diagnostic pathway.
• Experience resolving delays in diagnostic tests and outpatient appointments.
• Experience supporting post-treatment monitoring and rehabilitation.
• Experience developing referral systems so pre-referral tests and scans are actioned in advance.

Responsibilities

• Act as a dedicated contact for patients after a new cancer diagnosis, helping them access information and support.
• Coordinate care across health and social care services for frail or elderly patients and contribute to the NHS England Early Cancer Diagnosis Designated Enhanced Service.
• Support practices to improve cancer services and screening uptake, including action plans and tracking non-responders.
• Support MDT meetings and manage a caseload from discussions with the Cancer Clinical Lead and Clinical Care Coordinator.
• Monitor patients suspected or confirmed of cancer to ensure pathways meet Cancer Waiting Time Targets.
• Advise practices on cancer audit and referral review and support peer-to-peer learning events on diagnosis and screening data.
• Develop consistent safety netting approaches across the PCN for urgent investigations.
• Source training and resources from Cancer Alliance, Cancer Research UK, Macmillan and local partners.
• Build a library of PCN data packs and patient information in varied formats.
