Imperial College Healthcare NHS Trust is expanding its Cancer Clinical Nurse Specialist teams and inviting applications for Cancer Support Workers in Skin or Colorectal. You will join a CNS-led team providing personalised care across the patient journey, from diagnosis through treatment to survivorship or end of life care.

The post focuses on holistic assessment, information, and support, coordinating with the Macmillan Navigator Service and wider cancer team to ensure a single point of access

Formación

• Experience assessing holistic needs of patients or clients.
• Good understanding of cancer pathways and care coordination.
• Strong communication and teamwork abilities.

Responsabilidades

• Coordinate care with the Macmillan Cancer Navigator Service to ensure a single point of access for patients.
• Deliver personalised cancer care within the CNS team across 3 hospital sites.
• Provide information and support to enable patients and carers to navigate the health and social care system.
