Job summary

Communities in Torbay, Devon, seeks a Support Worker (Peripatetic) to support CFO Evolution participants across custody and community settings. You will manage caseloads, conduct assessments, deliver tailored interventions, and help with housing, substance misuse, and health services, promoting reintegration and safeguarding.

The role requires travel across the South West cluster, with 37 hours per week, on a long-term FTC to end of 2028.

Qualifications

• Willingness and flexibility to travel across the cluster/region and work in both community and custodial settings.
• Ability to manage caseloads and coordinate risk with safeguarding focus.
• Strong communication and engagement skills with diverse participants and stakeholders.

Responsibilities

• Manage a caseload of participants across custody and community settings.
• Carry out pre-release work, assessments, and create tailored action plans with interventions.
• Record participant data accurately to meet contractual and financial requirements.
• Meet individual and team performance targets.
• Ensure safeguarding of adults and children and report changes in risk to the Probation Service.
• Deliver assessments and activities to enable positive change and reintegration.
• Collaborate with peer mentors and volunteers to connect participants with housing, substance misuse support and health services.
• Participate in learning and promote best practices within the team.
