Job Summary

MMWF PCN is recruiting a clinical data and patient support officer to work across four GP practices in the Northampton area. The role focuses on generating and analysing clinical reports, managing patient appointments, supporting clinical initiatives and maintaining accurate clinical records.

Responsibilities
  • Run reports from SystmOne and other clinical systems (training will be provided).
  • Book patient appointments and manage patient clinical records with accurate read coding and data entry.
  • Work with GP teams and practice support staff to deliver administrative support.
  • Generate appointment data reports to understand capacity and demand.
  • Support patients to book appointments as part of projects and programmes (Spirometry, Mental Health, extended access/community clinic).
  • Help patients manage their needs by answering queries, making appointments and providing clear written or verbal information.
  • Support or manage clinics, including monitoring services and staff rotas.
  • Help patients gain access to self‑management education courses and peer support/interventions.
  • Support the coordination and delivery of multidisciplinary meetings.
  • Adhere to organisational policies and procedures, guided by occupational policies and procedures in primary and secondary care.
  • Work independently, plan your own workload and seek guidance from the line manager and colleagues.
  • Use clinical systems for record keeping, audit and evaluation.
  • Maintain confidentiality at all times.
