Leicestershire Partnership NHS Trust is seeking Service Coordinators for the Single Point of Access at New Parks Health Centre. You will handle telephone calls, e-referrals and emails to coordinate patient care across pathways.

The role focuses on ensuring patients see the right person at the right time and place, while maintaining confidentiality and accurate record-keeping. Applicants should be prepared to work as part of a team, support ongoing learning, and adapt to flexible working patterns

Responsabilidades

• Coordinate referrals, calls, emails and e-referrals within defined pathways.
• Act as point of contact for incoming referrals and liaise with professionals.
• Organise activities between organisations to prevent unnecessary admissions and support discharge.
• Maintain accurate records and ensure data confidentiality and security.
• Support learning and training requirements and participate in supervision.
