Job summary

Bristol NHS Foundation Trust is seeking a Patient Pathway Navigator to help ensure timely, patient-centred progression through orthopaedic pathways. You will monitor patient journeys, coordinate with clinical and admin teams, and help deliver safe and efficient care in a busy NHS environment.

The role requires strong data handling, communication and organisational skills, with the ability to respond to changing priorities and maintain accurate waiting list information for RTT targets.

Qualifications

• Educated to GCSE standard or equivalent.
• Experience in a busy multi-disciplinary team environment.
• Experience in an administrative role.
• Experience of working under pressure.
• Experience of working to fixed deadlines.
• Fluent English and clear written/verbal communication.

Responsibilities

• Accurate and consistent tracking of patient pathways on the Orthopaedic waiting list to ensure RTT targets.
• Use of clinical systems to determine a patient’s position and next steps on their pathway.
• Daily management of the patient tracking list (PTL).
• Updating clinical systems to reflect progress of patient pathways.
• Understanding non-admitted (NADM) aspects of pathways, including first appointments, diagnostics and follow ups.
• Booking patient appointments and liaising to expedite bookings in line with RTT targets.
• Understanding admitted (ADM) aspects of pathways for surgical waiting lists.
• Escalating delays to Orthopaedic management and admin teams to identify potential breaches.
• Contributing to departmental validation processes to ensure accuracy of waiting lists.
• Applying BNSSG Access Policy to patient pathways with confidence.
• Assisting Performance and Operations Manager with data reporting as required.
• Carrying out adhoc and general duties as required by the department.
• Developing positive relationships with Orthopaedic teams to progress patient pathways.
