A healthcare organization seeks an Op RESTORE Referral Coordinator to support veterans with their service-related physical health needs. This remote role requires the candidate to engage with GP practices and NHS Trusts, ensuring clinical information is managed effectively. The role of Op RESTORE Referral Coordinator provides specific support to veterans of the Armed Forces who have service‑related physical health needs.
This role is part of Op RESTORE which is a national service to provide support for veterans with service‑related physical health issues. The service provides navigation to the veteran and clinical signposting through NHS and veteran services.
The successful candidate will be linking with GP practices, NHS Trusts and social prescribers to ensure clinical information is available regarding veteran’s service‑related physical health needs and interrogating the clinical information to develop any onward clinical plan.
In conjunction with the Pathway Navigators, a holistic approach is applied to every veteran referred into the service to ensure their health and wellbeing needs are being supported.
The successful candidate can be based anywhere in England – this is a home‑working role with travel expected every few months to London or other parts of England, so the candidate will have adequate provision at home for an office to undertake confidential calls.
