Job summary

Hampshire County Council in Totton is seeking a qualified Occupational Therapist with HCPC registration to join our Reablement Service on a permanent, hybrid basis. You will carry out home visits, develop tailored intervention plans and review progress to help service users regain independence.

This role involves multidisciplinary teamwork, coordinating equipment and completing assessments with patient-centred care. Weekend rotation and bank holiday work are required.

Qualifications

• Occupational Therapy qualification and professional registration with HCPC.
• Understanding of the statutory duties of local government in social care and the social care model.
• Some experience of supervising staff.
• Experience in assessment, planning and reviewing of case work.
• Ability to work independently and collaboratively, escalating cases as required.
• Commitment to demonstrating our values and behaviours.

Responsibilities

• Carry out home visits to deliver holistic assessments and develop tailored intervention plans.
• Review progress and maintain records to support continuity of care.
• Work within the multi-disciplinary Reablement Service to promote independence.
• Coordinate equipment provision and safe living adaptations.
• Conduct triage and proportionate care reviews to allocate resources.
