Job summary

Hampshire County Council in the Totton area is seeking a qualified Occupational Therapist with HCPC registration to join the Reablement Service. The role focuses on home visits, holistic assessments and tailored intervention plans to help people regain independence.

The position is permanent, 37 hours per week, hybrid working with travel across West Hampshire. Weekend rotation and some bank holidays apply; closing date 18 November 2026.

Qualifications

• OT qualification and HCPC registration.
• Understanding statutory duties of local government in social care.
• Experience supervising staff.
• Experience in assessment, planning and reviewing case work.
• Ability to work independently and collaboratively.

Responsibilities

• Carry out home visits and holistic assessments.
• Develop tailored intervention plans and review progress.
• Work within the Reablement Service for short-term support.
• Undertake moving and handling risk assessments.
• Coordinate provision of equipment to support independence.
• Maintain accurate records and documentation.
• Support a seven-day service with weekend rotation.
