Job summary

Hampshire County Council is seeking a qualified Occupational Therapist (HCPC registered) to join our Reablement Service. You will conduct home visits, develop tailored intervention plans, and review progress to promote independence for service users.

You will work in a multidisciplinary team, undertake risk assessments, coordinate equipment, and document outcomes to support continuity of care. Weekend work is required on a rotational basis.

Qualifications

• HCPC registration is required.
• Experience delivering home visits and multidisciplinary teamwork.
• Strong assessment and documentation skills.

Responsibilities

• Undertake moving and handling assessments and reviews to identify needs and provide guidance on safe practices.
• Conduct home environment assessments to improve safety and accessibility.
• Arrange and coordinate equipment provision to support independence.
• Complete triage assessments to prioritise needs and allocate resources.
• Carry out proportionate care reviews to ensure plans remain appropriate and person-centred.
• Promote safe independent living through assessment and tailored recommendations.
• Maintain accurate records and documentation for continuity of care.
• Undertake bed rail risk assessments to ensure safety and wellbeing.
• Work seven days a week with weekend rotation (approx 1 in 8) and some bank holidays.
