Job summary

Hampshire County Council is seeking a qualified Occupational Therapist with HCPC registration to join its reablement service. You will undertake moving and handling assessments, home environment reviews, and coordinate equipment to support safe, independent living in the community.

You will carry out triage and care reviews, maintain records, and contribute to a seven-day service with weekend rotations. Experience in supervision and local social care duties is valued.

Qualifications

• Occupational Therapy qualification and HCPC registration are essential.
• Knowledge of local government social care duties is expected.
• Experience supervising staff is desirable.
• Experience in assessment, planning and reviewing case work.

Responsibilities

• Undertake moving and handling assessments and provide expert guidance on safe practices.
• Conduct home environment assessments to support accessible living spaces.
• Arrange and coordinate equipment provision to maintain independence.
• Complete triage assessments and allocate resources effectively.
• Carry out proportionate care reviews to ensure plans remain appropriate.
• Promote safe, independent living through assessment and tailored advice.
• Maintain accurate records and documentation to support continuity of care.
• Conduct bed rail risk assessments to ensure patient safety.
• Participate in a seven-day service with weekend rotation (approx. 1 in 8 weekends) and some bank holidays.
