Job summary

Hampshire County Council is seeking a qualified Occupational Therapist with HCPC registration to join its reablement service. You will undertake moving and handling assessments, home environment reviews, and coordinate equipment to support safe, independent living in the community.

You will carry out triage and care reviews, maintain records, and contribute to a seven-day service with weekend rotations. Experience in supervision and local social care duties is valued.

Qualifications

• Occupational Therapy qualification and HCPC registration are essential.
• Knowledge of local government social care duties is expected.
• Experience supervising staff is desirable.
• Experience in assessment, planning and reviewing case work.

What you’ll do:

• Undertake moving and handling assessments and reviews to identify individual needs and provide expert guidance on safe practices.
• Conduct home environment assessments to support safer and more accessible living spaces.
• Arrange and coordinate the provision of equipment to help individuals maintain independence and wellbeing.
• Complete triage assessments, prioritising needs and ensuring resources are allocated effectively.
• Carry out proportionate care reviews to ensure support plans remain appropriate, effective, and person-centred.
• Promote safe and independent living within the community through skilled assessment, advice, and tailored recommendations.
• Maintain accurate and up-to-date records, assessments, and documentation to support continuity of care.
• Undertake bed rail risk assessments, ensuring the safety, comfort, and wellbeing of service users
• Contribute to a service that operates seven days a week, with weekend working required on a rotational basis (approximately 1 in 8 weekends) and some bank holidays.

What we’re looking for:

• Occupational Therapy qualification and professional registration with HCPC.
• Understanding of the statutory duties of local government in social care and the social care model.
• Some experience of supervising staff.
• Experience in assessment, planning and reviewing of case work.
• Ability to work independently and collaboratively, escalating cases as required.
