Job description

Wehave an exciting opportunity within our Childrens IBD and PN Virtual wardSpecialist nursing service. Working closely with families, consultants, localand regional services, and community partners to provide high quality patientcare. Thepost holder will be responsible for managing and delivering care and supportto a caseload of children aged 0-18 years who have IBD and also patients onour PN and IBD virtual ward. The role includes teaching and supportingfamilies to care for their children who require complex medications. As well as participating in MDT clinicsalongside the consultant gastroenterologist and specialist dietitian, thepost holder will support the teams development and improvement of practiceand guidelines.

Excellentorganisational and communication skills are key elements for this positiontogether with a genuine compassion and commitment to care for children andfamilies who have a chronic illness.

Awillingness to undertake on the job training and experience of teaching/auditand research is desirable. The nutrition, gastroenterology and surgical teamwill provide support and training.

Main duties of the job

Thepost holder will support children and families with Inflammatory BowelDisease. Working alongside other IBD clinical nurse specialists and thewider MDT to provide telephone and face to face support and advice. Thisinvolves managing your own case load, providing nurse led telephone clinics,face to face clinics, supporting children and their families through a newdiagnosis, on-going treatment, flare up of symptoms plus more.

Inorder to provide a high level of ongoing care the post holder will attendregular meetings across a variety of MDTs to ensure their patients receivethe appropriate care at the right time. It will be the post holdersresponsibility to manage their case load and to feedback any outcomes frommeetings to the child and family.

Therewill be opportunity for prescribing should the post holder hold anindependent non-medical prescribing registration.

About us

The post holder will work closely with theIBD CNS team and will work alongside the gastroenterologists, specialistdietitians, MDT coordinator and surgeons.

TheIBD CNS team sit within the Childrens Nutrition and IBD CNS service and willwork alongside the nutrition nurses.

Supportwill be provided by the CNS Team Leader, which includes preceptorship period,regular 1:1 meetings, team meetings, and appraisal for the upcoming year

Thereare weekly MDT teaching sessions which aid the growth and development of theteam which in turn aids the development of the services the team provide.

Abackground in the systems we use at LTHT would be advantageous howeversupport and induction will be provided.

The job holder will provide training andeducation to staff groups, patients and families/carers.

  • Support patients and their families, communicating sensitive condition related information, ensuring they receive required information to enable them to participate in their care delivery.

  • Act in accordance with the relevant professional code i.e. NMC
  • Fulfil the requirements as directed by the Nursing and Midwifery Council to maintain and revalidate registration.
  • Raise concerns with regards to risk, danger, malpractice or wrong doing by following the LTHT Whistleblowing policy and supporting processes
Experience
  • Provide high quality care as an expert practitioner
  • Participates in the setting, monitoring and improvement of standards and quality of patient care
  • Assess, plan, implement and evaluate care and treatments using specialist clinical knowledge and clinical skills.
  • Organises own clinical workload
  • Make clinical assessments and treatment decisions within clearly defined policies and protocols
  • Supervise clinical staff to promote the delivery of excellent clinical standards
Qualifications
  • Registered Nurse (RN Adult for adult clinical areas or RN Child for childrens areas (Level 1 or 2)
  • Recognised teaching/assessing qualification or equivalent experience of teaching and assessment
  • Evidence of continued professional development
  • Degree Level Education or evidence of working towards
