Job summary

HCRG Care Group is seeking a Neurodevelopmental Support Practitioner based at White Lodge. You will provide early and ongoing support to children, young people and families on the neurodevelopmental pathway, acting as a care navigator and first-line practitioner, delivering telephone and face-to-face support, and assisting with triage and waiting list reviews.

The role requires travel across the locality; a full driving licence and access to a car are desirable.

Qualifications

• Experience supporting children, young people and/or families in health, education, social care or community settings.
• Experience providing direct or telephone-based support.
• Strong communication, organisation and time management skills.
• Ability to maintain accurate electronic records.
• Understanding of confidentiality and safeguarding responsibilities.

Responsibilities

• Be the first point of contact for families accessing the neurodevelopmental pathway.
• Review referrals and support waiting list management for accuracy.
• Provide telephone and virtual support with guidance and information.
• Support development of family resources, including website content and information packs.
• Coordinate follow-up appointments and maintain accurate records of interactions.
• Signpost families to appropriate services regardless of diagnostic outcome.
• Collaborate with clinical and administrative teams to support patient progression.
• Communicate sensitively to reduce uncertainty and distress for families.
• Escalate concerns and work within safeguarding, governance and professional boundaries.
