Midlands Partnership NHS Foundation Trust in Staffordshire is seeking a Neurodevelopmental Pathway Navigator to offer needs-led support and signposting for families and young people within the autism and ND pathways.

The role involves liaising with multidisciplinary teams, providing information and advice, and being the main contact for families on ND pathways. Based at Anglesey House, Rugeley, with some potential home visits across South Staffordshire.

Formación

• NVQ Level 3 Health & Social Care or equivalent experience.
• Knowledge of autism gained through formal training or experience.

Responsabilidades

• Provide guidance to families to access timely support and understanding of the assessment pathway process.
• Keep families informed of the support and options available to them and help them make informed choices on how they and their child or young person are supported.
• Support understanding of autism and other ND assessment processes with external agencies, partner organisations and referrers.
• Help families understand ND pathways through information and connection to local communities.
• Be the main contact for families with a child on Autism and other ND pathways and tailor contact to needs.
• Meet regularly with supervising clinician to review clinical issues and with team leader for managerial support.
• Work independently with children, young people and families in various settings.
