Northamptonshire Mind is seeking a Mental Health Outreach Support Worker to provide early mental health and emotional wellbeing support within the Age Well Teams. The role collaborates with Primary Care Network partners to help older adults in care homes and at home.

The role provides the opportunity to work innovatively with a range of partner organisations to enhance the experience of older adults living in care homes and their own home. The Age Well Teams provide early multi agency support for emerging needs, including Frailty clinics and care home teams and are working in partnership with Northamptonshire Mind to include mental health support for their patients.
