A primary care provider in Bristol seeks Mental Health and Wellbeing Coaches to support individuals with emotional challenges. The role involves managing patient caseloads, developing personalized support plans, and collaborating with Social Prescribing Link Workers for holistic care. Ideal candidates will have NVQ Level 3 in health and social care and a background in health coaching or counselling, with a competitive salary of £25,000 to £30,000 per year.
We are creating a small team to support patients with emotional and mental health needs appropriate for support in primary care in South-East Bristol. The Mental Health and Wellbeing Coaches will work with a registered Mental Health Practitioner to support adults who are accessing support for their mental health needs.

We are recruiting up to two whole time equivalent posts, which can be full or part time.
The Mental Health and Wellbeing coaches will manage and prioritise a caseload of patients referred from Primary Care. This will include people with mental health needs who do not meet the criteria for IAPT, people with serious or significant mental illness who are relatively stable, and those diagnosed or presenting with traits of personality disorder who will benefit from being signposted to services which can best meet their needs.
You will work with patients to develop a personalised support plan, and provide coaching and motivational support through multiple sessions to help them set and meet goals. You will also work with our Social Prescribing Link Workers to connect patients to appropriate community based activities to support their health and wellbeing.
You will work in a non-judgemental way, and will adhere to a code of ethics in line with the NHS England Health Coaching Implementation and Quality Summary Guide.
You will need to be resilient and well organised to manage a demanding case-load.
Connexus Primary Care Network brings together five GP Practices serving the population of Brislington, Knowle and Stockwood in South-East Bristol.
These new posts will work as part of a small team with a registered Mental Health Practitioner and our Social Prescribing Link Workers. You will work out of one or more of the surgeries, or in local community venues.
We offer a competitive rate of pay, with paid overtime. 25 days annual leave (plus bank holidays, pro-rata for part-time). You will receive regular clinical supervision and opportunities for training and development.
The purpose of this role is to provide personalised support to patients of Connexus PCN tohelp them take control of their mental health and wellbeing, live independently,improve their health outcomes and maintain a healthy lifestyle, usingbehavioural change techniques.
The postholder will:
Manage and prioritise a caseload inaccordance with the health and wellbeing needs of the population, taking anapproach which is non-judgemental and which considers the whole person
Provide personalised support toindividuals, their families and carers to support them to be activeparticipants in their own healthcare and wellbeing
Coach and motivate patientsthrough multiple sessions to identify their needs and set goals and supportingthem to achieve these
In partnership with externalorganisations, develop and embed integrated care plans which ensure that theneeds of the patient are met
Work closely in partnership with the Social Prescribing Link Workers to connectpatients to community based activities which support them to take increasedcontrol of their health and wellbeing and to identify and work alongside peoplewho may need additional support, but are not yet ready to benefit fully fromsocial prescribing
You will need to be confident in using GP clinical systems to manage referrals and maintain accurate and timely patients records.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
