Responsibilities

• Liaison with local authority referrals to match new packages of care.
• Set up service users' packages on rostering system (People Planner).
• Schedule service visits and carers' working rotas.
• Communicate with frontline staff about service delivery.
• Participate in out-of-hours on-call and report to Service Manager.
