Job summary

Call-In Homecare Limited in South Lanarkshire is seeking a Homecare Coordinator to schedule service users' care and rosters, and to liaise with local authorities to ensure high-quality service delivery. The role includes on-call duties and travel as needed.

The successful candidate will bring care coordination experience, strong people management and communication skills, and a valid driving license with knowledge of South Lanarkshire.

Qualifications

• Experience in Health & Social Care with care coordination preferred.
• Excellent people management and communication skills.
• Proven ability to manage a dynamic, high-quality care service.
• Full driving license and own transport.
• Good knowledge of South Lanarkshire area.
• SVQ Level 3 in Health and Social Care or willingness to work towards it.

Responsibilities

• Liaise with local authority referrals team to match new packages of care.
• Set up service users' package of care on People Planner rostering system.
• Schedule service users' care visits.
• Schedule Homecare working schedules.
• Communicate with frontline staff about service delivery.
• Take part in the branch office's out of hours on-call.
• Report effectively to the Service Manager and colleagues.
