Qualifications

• Experience managing a caseload of individuals with complex needs.
• Understanding of key legislation relevant to case management.
• Minimum 2 years’ experience as a Case Manager.

Responsibilities

• Correspond regularly with clients to manage their cases.
• Conduct Initial Needs Assessments and report findings.
• Organise workload on a daily, monthly, and annual basis.

Skills

Excellent organisational skills

Excellent written and verbal communication skills

Ability to undertake an Enhanced DBS Check

Education

Current registration with HCPC as an Occupational Therapist or NMC as a Registered Nurse

Minimum 5 years practice as a Professional Registrant

Member of BABICM, VRA or CSMUK
