Job summary

University Hospitals Coventry and Warwickshire NHS Trust is seeking a Pathway Navigator to support the Liver Health Checks ODN and regional HCC surveillance. Working with a multi-professional team, you will coordinate patient journeys, maintain tracking lists, and ensure surveillance appointments and tests happen on schedule for patients with cirrhosis.

You will provide telephone advice, help signpost to services, and support data quality, governance and audit within the Liver Cancer

Qualifications

• Degree in Nutrition and Dietetics
• HCPC registration
• Postgraduate master's level qualification or equivalent evidence
• Documented evidence of continuous professional development related to specialist area
• Clinical Supervisory Training (students)

Responsibilities

• Escalate issues and breaches of waiting times standards.
• Communicate information that is complex and sensitive pertaining to patients with diagnosed or suspected liver cancer.
• Responsibility for managing own time and ensuring tasks are completed in the required timeframe.
• Responsible for collating and updating patient information on the relevant databases in line with local policy, to ensure efficient pathway progress.
• Working with the operational team and clinical team to analyse target surveillance breaches and report issues to the ODN management team.
• Support and contribute to audit processes, governance, research, clinical research trials and service development.
