Responsibilities

• Support scheduling of Liver Health Checks clinics across the Operational Delivery Network (ODN).
• Maintain a Patient Tracking list for standard of care patients in surveillance.
• Coordinate patient referrals from the community Liver Health Checks CNS to Hepatology and radiology at the Trust, per pilot timings.
• Coordinate and provide administrative support to clinical teams.
• Maintain and populate the Patient Tracking list.
• Identify next steps in the patient’s surveillance pathway to meet target dates.
• Develop understanding of milestones in Liver Cancer surveillance and NICE guidance.
• Provide basic telephone advice regarding appointments.
• Make outbound preplanned calls to remind patients of upcoming surveillance scans and diagnostics.
• Print or email patient information and signpost to relevant web-based information or support services.
• Monitor patients on surveillance pathways and proactively resolve adherence issues to surveillance dates.
• Promote engagement and re-engage patients after a DNA.
