Job summary

University Hospitals Coventry and Warwickshire NHS Trust is seeking a Pathway Navigator to support the Liver Health Checks programme. The role focuses on coordinating care for patients with cirrhosis, managing surveillance pathways, and ensuring timely scans and follow-ups within a national cancer strategy framework.

Under supervision, you will liaise with multi-disciplinary teams, maintain patient tracking, and promote patient engagement in surveillance pathways across the network.

Qualifications

• Degree in Nutrition and Dietetics with HCPC registration is essential.
• Postgraduate level evidence of clinical dietetic development and CPD is desirable.
• BDA membership and teaching/leadership training are beneficial.

Responsibilities

• Schedule Liver Health Checks clinics across the ODN.
• Maintain a Patient tracking list for current surveillance.
• Coordinate referrals from community CNS to Hepatology and radiology within timelines.
• Provide administrative support to clinical teams and maintain data accuracy.
• Identify next steps in the surveillance pathway and milestones per guidelines.
• Provide basic telephone advice regarding appointments and reminders.
• Print or email patient information and signpost to relevant services.
• Monitor adherence to surveillance dates and re-engage patients as needed.
