Job summary

Sunderland Care and Support Ltd invites applications for a Care Co-ordinator (Level 1) in the Learning Disabilities Team in Sunderland. Deliver person-centred care and support to customers with respect, dignity and empowerment.

The role involves helping with meals, domestic tasks, medication administration, money management, community participation and planning activities. Training, induction and career development are provided.

Qualifications

• Level 3 QCF Health and Social Care or equivalent, or ability to complete within 2 years of commencement in post
• English and Maths Functional Skills at level 1
• Experience of working in a direct support capacity with vulnerable adults
• Experience of providing high standards of customer care
• Experience working as part of a team and on own initiative
• Basic IT skills
• Knowledge of health and safety at work and ability to carry out checks
• Understanding of safeguarding policy and empowerment of customers
• Ability to provide personal or practical interventions to range of customers
• Ability to carry out intimate tasks without supervision
• Ability to liaise with colleagues and management
• Ability to mentor or support colleagues and apprentices
• Excellent written and verbal communication
• Ability to work flexibly and use own initiative

Responsibilities

• Helping with preparation of meals and drinks
• Support with domestic household tasks
• Administer and record medication
• Support with money management
• Encourage community trips and participation
• Support with shopping and meeting friends/family
• Support with nutrition and health and safety
• Cleanliness and intimate personal hygiene
• Provide emotional support
• Use active support techniques and positive behaviour support
• Plan holidays with customer (additional payment)
• Duties may change to reflect needs of customer/service
