Job summary

Sunderland Care and Support Ltd is recruiting two Care Co-ordinators (Level 1) for our Day Service. The role focuses on delivering person-centred care to adults with complex needs, promoting independence and wellbeing on-site in a supportive, values-driven environment.

Salary range £27,274 to £29,071 per year (pro rata if less than 37 hours). DBS check funded, regular supervision, and clear progression paths are provided. Two references and enhanced DBS may be required.

Qualifications

• Essential: Level 3 QCF Health and Social Care or equivalent or complete within 2 years.
• Experience in direct support for adults with disabilities.
• Experience delivering high standards of customer care and working in a team.

Responsibilities

• Provide person-centred care and support to adults with complex needs.
• Coordinate care plans and liaise with multi-disciplinary teams.
• Assist with daily living tasks and empowering independence.
