Sunderland Care and Support Ltd is seeking a Care Co-ordinator (Level 1) for our Autism Team in Sunderland. The role involves delivering person-centred support to adults with complex needs, sometimes requiring personal care and active support to enable independent living.

The post is permanent, full-time, on-site, with a salary of £27,274 to £29,071 per year and includes DBS costs covered, a pension, and regular supervision.

Responsibilities

• Helping with preparation of meals and drinks.
• Support with domestic household tasks
• Administer and record medication
• Support with money management
• Encouraging community trips and facilitating customers participation
• Support with household and personal shopping
• Support with meeting friends and family
• Support with high standards of nutrition
• Adhere to health and safety legislation
• Cleanliness and intimate personal hygiene
• Opportunities for employment or education
• Provide emotional support.
• Active support techniques and positive behaviour support techniques to increase skill level and achieve set outcomes (training will be provided)
• To support customer to plan holidays and support them if needed (additional payment)
