Montana Home Care is seeking an experienced Care Coordinator to lead a Falkirk-based team delivering person-centred care in clients’ homes. You will manage staff rotas, conduct risk assessments, audits and ensure health and safety compliance, while deputising for the Service Manager when required.

The role requires SVQ Level 3 in Health & Social Care, at least 2 years in a similar setting, strong leadership and communication skills, confidentiality and safeguarding commitment, and a valid UK

Formación

• SVQ Level 3 in Health & Social Care or equivalent.
• Minimum of 2 years’ experience in a similar social care role.
• Proven supervisory and leadership experience.
• Excellent communication and interpersonal skills.
• Commitment to confidentiality and safeguarding.
• Driver with access to a vehicle.

Responsabilidades

• Lead and support staff to deliver high-quality, person-centered care.
• Manage day-to-day operations including staff rotas, risk assessments, audits and H&S compliance.
• Supervise and develop Support Workers; conduct appraisals and performance management.
• Ensure the Service uses our Person-Centered Support and Enablement model.
• Liaise with social workers, families and external agencies to achieve best outcomes.
• Deputise for the Service Manager when required.
