Montana Home Care is seeking an experienced Care Coordinator to join our dedicated team in Falkirk and the surrounding areas. This role offers a leadership position in a care-at-home service, coordinating staff to deliver high-quality, person-centered care.

You will manage day-to-day operations, supervise and develop staff, ensure safeguarding and compliance, and liaise with social workers and families to achieve the best outcomes.

Qualifications

• SVQ Level 3 (or equivalent) in Health & Social Care – Essential.
• Minimum of 2 years’ experience in a similar role within a social care setting – Essential.
• Proven supervisory and leadership experience with the ability to inspire and develop a team.
• Excellent communication and interpersonal skills, with the ability to build trust and effective relationships.
• Professional, resilient and adaptable, with strong problem-solving skills.
• Commitment to confidentiality, safeguarding and organisational values.
• Driver with access to a vehicle – Essential.

Responsabilités

• Lead and support staff to deliver high-quality, person-centered care to people in their own home.
• Manage day-to-day operations including staff rotas, risk assessments, audits, and health and safety compliance.
• Supervise, mentor and develop Support Workers with appraisals and performance management.
• Ensure our Person-Centered Support and Enablement model of care is embedded throughout the service.
• Liaise with social workers, families and external agencies to achieve the best outcomes for the people we support.
• Deputise for the Service Manager when required.
