Responsibilities

• Lead, motivate and support a team of Care Coordinators.
• Set clear expectations, priorities and performance standards.
• Monitor team KPIs, productivity, service continuity and workforce utilisation.
• Lead team meetings, daily huddles and operational planning.
• Coach team members and support capability, development and succession planning.
• Identify and address performance and capability concerns.
• Oversee workload allocation and workforce coverage.
• Proactively manage vacant visits, staffing risks and service disruptions.
• Act as the first escalation point for complex coordination and service delivery matters.
• Monitor quality, compliance, incidents, complaints and operational risks.
• Use service data and team feedback to identify improvement opportunities.
• Lead approved process improvements and support organisational change.
• Build strong relationships with clients, families, support workers, managers and external stakeholders.
• Promote a culture of accountability, collaboration, customer focus and continuous improvement.
