Job summary

Libertatem Healthcare Group is seeking a proactive Care Coordinator to join our domiciliary care team in the UK. You will develop and manage care schedules, ensure continuity of care, and maintain clear communication with clients and families.

Lead team meetings, recruit and match support workers, coordinate resources across care packages, and mentor staff while upholding high standards of care. Own transport preferred; generous leave and wellbeing benefits are included.

Qualifications

• Proven experience in domiciliary care or similar setting.
• Excellent customer service and communication skills.
• Strong logistical planning and problem-solving abilities.
• Calm under pressure with a collaborative mindset.
• Experience mentoring or coaching team members.
• Own transport is essential.

Responsibilities

• Develop and manage care schedules tailored to individual needs.
• Ensure continuity of care during staff absences.
• Maintain strong communication with clients and families.
• Lead team meetings to foster collaboration and problem-solving.
• Recruit and match support workers to clients.
• Work with HR and Clinical Leads to resolve staff issues.
• Support ongoing training and development.
• Liaise with internal and external stakeholders.
• Supervise care and medication standards.
• Maintain accurate records and report on client progress.
• Plan and deploy resources across care packages.
• Assist with scheduling, billing, payroll, and on-call duties.
