Job summary

Derbyshire Community Health Services NHS Foundation Trust is seeking an experienced Band 6 Community Physiotherapist to lead and develop the team at Ilkeston Hospital and across Bennerley Fields Centre and Ripley Hospital.

You will manage a varied caseload including neurological, orthopaedic, respiratory and elderly rehabilitation, within an integrated team and MDT, while supporting junior colleagues and delivering patient-centered care.

Responsibilities

• Manage a wide array of clinical conditions including neurological, orthopaedic, respiratory and elderly rehab.
• Work as part of an integrated MDT with Physiotherapists, Occupational Therapists, Advanced Therapy Support Workers, Therapy Support Workers, and alongside Social Care, Nurses, Care Coordinators, ACPs and other specialist services.
• Manage the clinical caseload and attend regular therapy meetings.
• Undertake individual and joint therapy assessments and treatment sessions with members of the MDT where appropriate.
• Deliver rehabilitation with a focus on patient-centred goals and facilitating discharge home.
