Job summary

Midlands Partnership NHS Foundation Trust in Staffordshire is seeking an Assistant Therapy Practitioner to join the Home First service in the South of Staffordshire. You will deliver high quality rehabilitation to adults with diverse health needs and work alongside qualified therapists to provide holistic care in patients' homes.

The role requires engagement with complex cases, independence within scope of practice, and participation in a shift pattern including Bank Holidays.

Qualifications

• NVQ Level III in Health and Social Care or Foundation Degree in Health and Social Care.
• GCSE Grade C/4 in English and Maths.
• Evidence of successful study at Level 3 (e.g. A level, NVQ, BTEC) or completion of MOOC to be eligible to commence this course.

Responsibilities

• Manage a caseload of adults in close liaison with the Home First Team.
• Undertake independent, holistic assessments in patients' normal place of residence.
• Carry out rehabilitation programmes set by qualified therapists.
• Discharge appropriate patients and complete onward referrals and discharge summaries.
