Job summary

Norfolk Community Health and Care NHS Trust in Norwich is seeking an experienced Therapy Assistant Practitioner to join the Central Norfolk Home First Hub. The role involves triaging therapy referrals and supporting discharge to assess pathways across a 7-day operation, including home and care home visits.

You will work within an integrated multidisciplinary team and rotate across community settings, working closely with OT, Physio, SW colleagues, and Integrated Care Coordinators.

Qualifications

• Experienced and reliable Therapy Assistant Practitioner to support the Central Norfolk Home First Hub.
• Triage of therapy referrals for patients discharged from acute and community hospitals.
• Telephone assessments and review of individuals' abilities living at home or in care homes after discharge.
• Therapy visits in the community to complete initial assessments and treatments.
• Familiarity with Systmone, TCES Medequip, Liquid Logic (training provided).
• Ability to prioritise workload, communicate effectively, work in a team, and use initiative.

Responsibilities

• Triage referrals for Discharge to Assess pathways and allocate referrals.
• Deliver high standards of care and plan, implement and evaluate packages of care in patients' homes, care homes, and clinics.
• Facilitate early discharge and prevent unnecessary hospital admissions.
• Liaise with multidisciplinary teams and statutory/non-statutory agencies for integrated care.
